the question is not does NFPA require a smoke in the rest room
is it required by your local code which is not a simple question.
NFPA tells you how to do it not what do to.

to try and help you tell us about your situation.
ie. Is this an inspection, new install, type of building, sprinklered, type of occupancy, who is asking for it and where is this.

You need to refer to the guidelines published by the AHJ. In some areas buildings that are fully sprinkled, only electric, it, elevator and mechanical rooms reguire detectors. Some also require smoke or heat detectors in pantry's.

But NFPA state that to consider nuisance alarm due to steam, moisture and dust.

from my point of view toilets and wash rooms are moisture content area.hence its better to avoid the smoke detector in such areas.

generally smokes are not put in wash rooms, usually it is heats
I assume a wash room is a bathroom/restrooms
I had a job the fire marshal wanted smokes in the restrooms, so you just do it

if it is total coverage, every room is every room
