I had a board that was in OS maintenance mode and would not come out.
I called tech support, who said I needed to replace it.
OS (Operating system) maintenance mode - Alternates DS2/DS3, On DS7
((( you will not be able to web into the card if this switches are not set ))
Set dip switch SW5-6 to OFF.
Set dip switch SW5-7 to OFF.

evein i faced the same problem at one of site.I have shorted the jumper and reboted the card and also in integrated configuration tool i have selected picture perfect and after restart of PXN plus card and again changed it to secure perfect and it worked.
I don't know above two which worked out but my all the three controllers worked fine there after.
